Order Repeat Medication

You can order your repeat medication in one of three ways:

  • 1. Request online - Patient Access

    Patient Access is a website and mobile app which gives you access to a wide range of GP services, such as ordering repeat prescriptions; booking appointments; and access to your medical records*. You can download the app for free in Google Play or the App Store. To register for Patient Access, please complete the form below:

    *access to medical records is an advanced feature that requires additional authorisation and is for only 11 days, please only register for this if you require it as it will otherwise delay processing of your request.

  • 2. Nominated Chemist

    Ask your nominated chemist to order it for you

  • 3. Letter to the Practice

    Write a letter to Clapham Family Practice (we do not accept telephone requests for repeat medications).  Please note the practice cannot be held accountable for lost or delayed letters.

If you have a query or would like more information about your medicines, and your community chemist has not been able to answer it, Clapham Family Practice employs a team of pharmacists who can help. They have been specially trained to prescribe medicines and issue prescriptions.
